STEP 1:  DETERMINE HOME SCHOOL BOUNDARIES
Using the District Boundary Map verify that you do live in the boundaries serviced by the School District of Cudahy.  (If you are not sure which elementary school is your neighborhood school of attendance, please call the Administration Building 294.7400)

STEP 2:  COMPLETE THE NEW STUDENT ONLINE ENROLLMENT PROCESS (NSOE)

• Request a Username/Password

• Username and Password will be emailed to you instantly

• Use the new username (email address) and random password and begin completing the enrollment forms

  • Basic demographic information on family and each student

  • Medical Alert

  • Special Dietary Needs

  • Additional Parental Sign-offs

    • Student Handbooks

    • Directory information

    • Photo Release

    • Immunization Records Access  (only if from Wisconsin)

  • SUBMIT

    • Your information will be reviewed by district personnel and once verified you will receive an email with further instructions on how to complete the enrollment.

    • You should receive this within 2 business days... if not please call 414.294.7400 and ask about yours.

STEP 3:  VISIT YOUR SCHOOL/DISTRICT OFFICE TO VERIFY RESIDENCY & AGE
This step includes visiting the Registrar at the Administration Building (2915 E. Ramsey Avenue) to provide Proof of Residency, Proof of Age and Immunizations (non WI residents).
